Abstract

From April 2015 to 2016, sixty-eight accidents were recorded in the facility which translates to an expense of Php 201,730.85. In order to eliminate the occurrences of accidents in the facility, at the same time improving the workplace safety and health, a framework and an assessment tool was devised by this study. The framework consists of three phases: analysis of accident occurrences, risk and hazard identification risk and hazard assessment form or check sheet was derived from the results of the analysis of the phases. Items in the form were based from the results of the framework and the standards given by the policy-making bodies and other related studies. A safety assessment form was derived from the framework and then applied to HAZCHEM, Inc. Based from the use of the assessment form, several hazard control measures were recommended. The control measures suggested are under PPEs, material handling procedures, housekeeping policies, design of facility, safety personnel requirements and other technical requirements. Several measures were also proposed to sustain implementation of recommendations.
